When submitting the catastrophic rate, should we include materials?
INCLUDE AN INDUSTRY STANDARD PRICE SHEET PLUS ANY MARK-UP THAT YOU
WOULD CHARGE BUT DO NOT INCLUDE THESE COSTS IN THE BASE BID COSTS.
Regarding the USF discount, do we show our totals then the discount,
or do we just present our discounted rates?
SHOW THE PRE-DISCOUNTED RATE IN ALL SPACES OF SCHEDULE 1 WHETHER ELIGIBLE
OR INELIGLE EXPENSES.
Are construction costs eligible for USF discounts?
NO; LEASE COSTS AND "INSTALLATION COSTS" ARE FOR FCC-DEFINED COMMON
CARRIERS WHO PAY, AS WE UNDERSTAND, INTO THE UNIVERSAL SERVICE FUND. SEE

What are the delivery expectations?
DARK FIBER IS SUCCESSFULLY TESTED AND CERTIFIED AS USABLE AND MADE
AVAILABLE TO CIVICNET WITHIN 120-DAYS OF CONTRACT SIGNING OR, AS APPLICABLE,
FCC FUNDING SET-ASIDE (ESP. IN CASE OF LAN-RFP).
Can dark fiber be delivered as an ATM OC-3 service?
CIVICNET PREFERS TO PROCURE DARK FIBER AS OUTLINED IN ADDENDUM#1, ITEM#4
WITH THE INTENT OF "LIGHTING" IT WITH ITS OWN HARDWARE. CIVICNET REITERATES
ITS PREFERENCE TO EITHER A 99-YEAR LEASE OF THE DARK FIBER PLANT WITH AN
UP-FRONT PAYMENT OR A 1-5 YEAR LEASE-PURCHASE OF THE DARK FIBER PLANT OR
SOME PARTNERSHIP ARRANGEMENT OF MUTUAL BENEFIT THAT TAKES INTO COSIDERATION
THE AFORMENTIONED.
Will you accept any service type bids that will allow industry standard
interfaces (i.e. ATM, ethernet, NTSC video, DS-1 RJ48, etc.) versus a vendor
specific proprietary design?
CIVICNET SEEKS AN ALL ATM SOLUTION INTO EACH BUILDING; CIVICNET PLANS
TO DEPLOY ATM SWITCHES AT EACH LOCATION (SEE EXHIBIT 1-REVISED) FOR ATM
TO THE STUDENT DESKTOP AND ROUTERS TO ALLOW EXISTING LAN'S TO INTERFACE
AND "RIDE" THE ATM NETWORK.
Can you provide site by site information regarding the type of service
and interfaces you desire? (e.g. how many voice, data and video circuits
and types of interfaces.) If voice is required at each site, we need to
know the originating location.
CIVICNET HAS PURPOSEFULLY PLANNED FOR THE DEPLOYMENT OF ITS OWN ATM
HARDWARE (SEE LAN-RFP) IN ORDER TO INTEGRATE ITS VOICE, VIDEO, AND DATA
REQUIREMENTS. AS TO INTERFACING THE TELEPHONY PORTION OF THE NETWORK
WITH A TELCO C.O., CIVICNET WOULD INVITE PROPOSALS ADDRESSING THE SAME
WITH THE UNDERSTANDING THAT INITIAL DEPLOYMENT OF TELEPHONY (BOTH INTERNAL
AND EXTERNAL)—INCLUDING THE ACQUISITION OF A PBX TO SUPPORT THE TELEPHONY
NEEDS—IS PLANNED FOR THE FUTURE.
Would Civicnet consider extending the bid due date to allow time
to receive your answers and process the information?
NO. DUE TO SCHEDULING CONFLICTS, FCC-USF DEADLINES, GOVERNING BOARD
MEETING DATES, AND SCHEDULED SPRING HOLIDAY BREAKS, THIS WILL NOT BE POSSIBLE.
How do I go about obtaining an FCC-USF "SPIN" number?
FCC-USF "SPIN" NUMBERS MAY BE OBTAINED, WE UNDERSTAND, BY CALLING THE
FOLLOWING NUMBER: (973) 560-4440 THIS NUMBER WILL BE ANSWERED BY THE UNIVERSAL
SERVICE ADMINISTRATIVE CORPORATION (USAC), AN ADMINISTRATIVE AGENT OF THE
FEDERAL COMMUNICATIONS COMMISSION.
Is the Call Julie, Illinois locate service, part of the maintenance
requirement?
NO. HOWEVER, THE PLACEMENT OF SIGNS ALONG THE UNDERGROUND ROUTE IDENTIFYING
BURIED CABLE WHEN THE SAME IS NOT PART OF AN EXISTING PLANT WILL BE REQUIRED.
THE DISTANCE BETWEEN SIGNS AND THE COST OF THE SAME SHOULD BE SHOWN AS
AN "ALTERNATIVE ADD-ON COST". JULIE SERVICES MAY ALSO BE INCLUDED IN THIS
MANNER—IE., AS AN "ALTERNATIVE ADD-ON" COST; BUT IN NO CASE SHOULD EITHER
BE INCLUDED IN THE BASE BID COSTS.
Schedule 1 of the WAN-RFP suggests a 20-year Lease? How does this
reconcile with Addendum #1, Item 4 "Preferences."
IT DOESN'T. SEE NEW WAN-RFP SCHEDULE #1 BELOW.
Where do we attach "Alternative Add-on" costs for things that we
would propose that are beyond the four-corners of the Request for Proposals
but which do have a cost?
ATTACH "ALTERNATIVE ADD-ON COSTS" TO SCHEDULE #1 AND CLEARLY MARK THE
SAME "ALTERNATIVE ADD-ONS". SIMILARLY, IF YOU HAVE "ALTERNATIVE DEDUCTS"
FOR SAVINGS THAT MAY BE ACCRUED BY PROCEEDING IN A DIFFERENT MANNER, ATTACH
AND CLEARLY MARK THE SAME "ALTERNATIVE DEDUCT" AND COMPILE FOLLOWING SCHEDULE
#1.
If we have certain restrictions or covenants to be placed on the
use of leased dark fiber or on that ultimately purchased, where would be
list such restrictions and/or covenants?
ON SCHEDULE #2 AS PART OF YOUR SPECIFIC RESPONSE TO THE RFP.
What is Schedule #3 for?
SCHEDULE #3 IS DESIGNED FOR THE CIVICNET TO EASILY RECONCILE THE DIFFERENCES
BETWEEN THE PROVIDERS SCHEDULE #1 AND #2 COSTS AND PROPOSAL, RESPECTIVELY,
AND THOSE OF THE CIVICNET SPECIFICATIONS, RFP "FRONT-END" DOCUMENTS, AND
ALL THE ADDENDUMS ISSUED AND EXHIBITS POSTED UNLESS OTHERWISE CLEAR ON
THEIR FACE. FOR EXAMPLE, A RESPONDENT TO THE WAN-RFP UTILIZING AN EXISTING
NETWORK WITH SEVERAL "NEWLY INSTALLED" LATERALS WOULD SHOW THE SAME—IN
MAP FORM—AS PART OF THEIR SCHEDULE #2 RESPONSE. ON SCHEDULE #3 THE PROVIDER
WOULD SIMPLY REFERENCE THE NEW MAP. BY CONTRAST, IF THE PROVIDER WERE TO
INSTALL THE LATERALS AT A SPECIFICATION OTHER THAN THAT REQUESTED, THEY
WOULD NEED TO HIGHLIGHT THAT CHANGE, DOING SO ON SCHEDULE #3.

INSTRUCTIONS
In the spaces provided, enter the requested dollar figure. Append to
Schedule 2 your response to the Request for Proposal. List on Schedule
3 any and all "Exceptions" the service provider proposes in their response
(Schedule 2) to those sections of the RFP, generally, and the specifications,
applicable exhibits specifically. Use the "Section Title" of the RFP or
"Reference #" of the Specification Section to refer back to the document
when noting deviations. In NO instance will a proposal be considered if
any one of the items proposed is less than the minimums specified. Initial
the quotation sheet in the space indicated.

*Provider shall list total cost for 99-years recognizing payment would be made in an up-front lump-sum; This would not include Maintenance as the same is eligible for FCC-USF and "lump-sum" payments are prohibited. If provider proposes different procurement terms, the same should be noted on Schedules 2 and highlighted in Schedule 3. For comfort, Provider may substitute its own Schedule 1 so long as the same follows the format shown above: 5 columns by 7 rows of either instructions or responses.
