NOTE:
When an individual's performance indicates he/she
is likely to be changing from "informal"
to "formal", opportunities/options for
assistance will be made available for evaluatee.
As it is incumbent upon the evaluatee, said individual
will self-select options. The evaluator may recommend
specific options.
Options may include but not limited to peer coaching,
release time for observations, additional training/development,
and teaming with a colleague.

review of goal accomplishments or any additional

evaluation conference/sign-off would be conducted
no less than 30 days prior to end of the official

the summative conference, the evaluator will provide
the evaluatee a written evaluation which will
include the overall rating, confirmation of evaluation
status for the next evaluation cycle, and a narrative.
The narrative will include professional background
