Release Form

The following K-12 School Districts do not currently have an official web publishing policy, however, they are using some form of release form to address the concerns of posting student photos and names. In addition, many of these districts have an Acceptable Use Agreement, however it does not address the web publishing.

Highlights:

• A simple opt-out release form is available for parents who do not want their student's names/photos published on-line.
• Opt-out release form addresses all school activities/program such as birthdays, Halloween & Christmas parties, as well as posting of student photos on the web.
• Opt-out release forms are kept on file by classroom teachers
• Release forms are kept on a year-to-year basis and must be renewed each year.
